The fierce struggle about the bible of psychiatry

After 14 years work the new US handbook to mental disorders was now approved. But criticism to the new version is huge. Unnecessary or even dangerous are some changes.

"This is the saddest moment in my 45 years carrier as psychiatrist", wrote Allen Frances in December 2012 in the American science blog "Psychology Today". The reason: The association of American psychiatrists (APA) had just sanctioned the newly published diagnostic bible of psychiatry - the handbook with which doctors and psychotherapists determine psychic illnesses.

In May 2013 the DSM 5 ("Diagnostic and Statistical Manual of Mental Disorders") will now be published in the USA, including the first changes since 1994. Frances opinion about the new version of the standard work is devastating and very important.

Because the psychiatrist himself had been responsible as scientific chair for the handbook, the DSM 4, which is still valid. "My advice to clinics, the press and the broad public: Be sceptical and do not follow DSM 5 blindly", so his result.

For instance sex addiction was discussed for a long time.

First it was supposed to be counted to the behavioural addictions, then to be described as a completely new illness called hyper sexuality, and finally it was thrown out completely. Many experts would have liked to have it in the manual but the scientific basis was too thin.

In total, so the APA reported, the number of psychiatric illnesses will now remain the same, and only few change fundamentally. Nevertheless critics believe that some diagnoses have been softened up without sufficient scientific reasons.

They fear that the new rule work will make patients out of many healthy people.

Frances insists that there will be a drastic increase in diagnosed illnesses. "Painful experiences with forerunner DSMs teach: When just anything in the diagnostic system can be abused and changed into a fashion thing, then it will happen."

He has even written a book about it. In spring it is to be published, right to the printing of the DSM 5, under the title "Saving normal" ("Normal: Against the Inflation of Psychiatric Diagnoses").

Frances also writes: "DSM 5 will change outbursts of temperament into a psychiatric disturbance a confusing decision, which is based on the work of one single research group."

He sees the new diagnosis sceptically, says the doctor of the University Duisburg-Essen. "One has to see that one does not psychiarise children with a difficult temperament."

The DSM 5 has been sanctioned prematurely.

The manual is to fill up the budget.

Possibly the reasons for it lie the manual itself.

"Only for one reason the present draft is to go to the printers as fast as possible: to fill the gigantic hole in the finance budget of the APA." 25 million dollars the work at the DSM 5 has cost so far but after all the manual is also a bestseller over years as worldwide standard work provided it is printed.
